My guess is that you didn't install all the files from breqn in your TeX
tree, i.e.

   breqn.sty
   flexisym.sty
   mathstyle.sty
   cmbase.sym
   msabm.sym

Use the command 'kpsewhich' from the command line to see if TeX can find
the files, e.g.

   kpsewhich breqn.sty

etc.
