Revised reference manual at the Maxima web site

Hello,

I have regenerated the html and pdf versions of the Maxima
reference manual and copied them to the web site.
See http://maxima.sf.net/docs/original/maxima_toc.html
and http://maxima.sf.net/docs/original/maximareforig.pdf .

Please direct corrections and additions to the SF bug tracker,
under the category "Documentation", if you would.
Let's direct comments about stylistic issues to the mailing list.

I have applied a simple-minded css style file to the html pages.
There is essentially endless opportunity for tweakage here.
I slightly modified texi2html to generate references to the css.

Fwiw I hereby declare the texinfo docs ready for 5.9.2 release.
