300 dots per inch.  A PDF, JPG, or GIF would work,

It's called Perfect Binding, like a paper back book. I just checked 
lulu.com,  the
maximum sized 8.5 x 11 b/w book is 470 pages. We would have cut something
or break it into two books.

We add whatever profit we want. We get 80% of that, Lulu keeps 20%
