Yes. My advice is to avoid hyperlinks entirely.

How about generating a TeX macro for each index item
from index.hhk, which maps each item's name to the appropriate
hyperlink? If you decide to use hyperlinks after all, that is.

The current documentation system is a mash-up of various
document processing schemes, held together with the proverbial
duct tape & bailing wire. It is certainly not guaranteed to stay the
same for any length of time, although it has a certain
institutional stability about it.
